Hello EVPB Families,

We are going to extend the order form by 1 day. We would like to remind all families that you MUST submit this form by the end of the day TOMORROW - 6/23 - in order for your child to receive their items (this includes their water jug) in a timely manner. All other items and orders will not be ordered until after band camp begins. 

It takes 5 minutes or less to complete the form and you need the following information: your child’s shirt, shorts, and shoe size to complete the form. We also ask that you alert us of food allergies or any other pertinent information that we may need for your child’s success.

Hola familias de EVPB:

Ampliaremos el formulario de pedido un día. Les recordamos a todas las familias que DEBEN entregar este formulario antes del final del día de MAÑANA - 6/23 - para que sus hijos(a) reciban sus artículos (incluido la jarra de agua que necesitan) a tiempo. Los demás artículos y pedidos no se realizarán hasta después del inicio del campamento de banda.

Completar el formulario toma 5 minutos o menos y necesita la siguiente información: la camiseta, los pantalones cortos y la talla de calzado de su hijo(a). También les pedimos que nos informen sobre alergias alimentarias o cualquier otra información pertinente que podamos necesitar para el éxito de su hijo(a).
